I certify that I have been informed of the risks and safety precautions and rules associated with participating in the Campus Trash Audit (a.k.a. "Dumpster Dive") being held on the SUNY Buffalo campus on April 16, 1998, (rain date, April 23, 1998).
I further certify that I am in good health and have no known respiratory, cardiac or immune system illnesses or any other conditions or problems which could be aggravated by the stress of wearing protective equipment or by coming into contact with materials in the trash stream.
I agree to obey all safety precautions and rules.
I agree not to remove or read any materials found at the audit site.
I understand that I am participating in this event voluntarily and at my own risk. I absolve and hold harmless SUNY Buffalo and its agents, the State of New York, and all SUNY Buffalo employees and volunteers assisting with the audit of any liability in the event of accident, injury or illness.
