• Third party address(es) where the documents should be sent
• There are no fees for the services listed above.

Special Services:

Certification of Copy of Diploma: You must send us an 8.5''x11''copy of your original diploma. The OME will verify that it is a true copy of the original and will affix the UB seal on the copy.

Replacement Diplomas: Contact OME at (716) 829-2802 for a replacement application. A diploma can only be issued for a name change or if evidence shows that the original diploma has been irretrievably lost or destroyed. The fee is $20.00.
