Community Partners

 

The Primary Care Research Institute works collaboratively with a variety of local and national partners. By working with the community, we are better able to identify issues that can be addressed in the primary care setting. Listed below are some of the community partners we currently work with. If we have left off your group, please send us an e-mail to let us know.

Partner Organization Description
American Lung Association The American Lung Association is the oldest voluntary health organization in the United States, with a National Office and constituent and affiliate associations around the country.

Area Health Education Center (AHEC) The mission of the New York State Area Health Education Center (AHEC) System is to enhance the quality of and access to health care, improve health care outcomes and address health workforce needs of medically underserved communities and populations by establishing partnerships between the institutions that train health professionals and the communities that need them most.

Erie County Department of Health The mission of the Erie County Department of Health is to protect the public's health, to prevent disease and premature death through the promotion of healthy living and wellness, and to provide a safety net of services for those with limited access to healthcare.

Gold Choice Gold Choice PCMP IIA is a partially capitated Medicaid Managed Care Program utilizing the Physician Case Management Model for ensuring access to Primary Care and specialty Medical Services. Such providers consistently see the program as the Medicaid Managed Care Program of choice for individuals with a chemical dependency and/or mental health disability. Gold Choice provides specialized case management services that ensure that at risk populations have access to specialized medical services and that such services are coordinated with their chemical dependency and mental disability services.

Erie County Department of Mental Health The Department of Mental Health oversees mental disability services for over 48,000 Erie County residents each month through contracts with 42 community based agencies, including the Erie County Medical Center and Erie County Departments of Senior Services, Social Services and Youth Services.

The International Institute The Institute helps newly arrived refugees and immigrants find work, learn English and helps the local communities learn about other languages and cultures.

Jericho Road Family Practice Jericho Road Family Practice is a private family medical practice providing comprehensive medical services to the underserved West Side of Buffalo.

Journey's End Journey's End Refugee Services is a non-profit agency that offers an array of services to enhance the lives of refugees resettling into Western New York.

Healthcare For A Nation Dedicated to the introduction of the Seneca Nation of Indians, its facilities, services offered, hiring needs, culture and its people to to health care providers throughout the United States

The Toxic Waste Lupus Coalition The Toxic Waste/Lupus Coalition Project was established to explore the high risk of infection for the African American community to environmental toxic pollutants.

UNYNET Upstate New York Practice Based Research Network includes 60.5 full-time equivalent (FTE) clinicians, practicing at 42 Family Medicine sites in 8 counties of New York. UNYNET sites cover the spectrum from inner-city urban to suburban to small-city to rural.

The West Side Community Asthma Project The West Side Community Asthma Project is a partnership between the residents of Buffalo's West Side and the University at Buffalo. Our ultimate goal is to improve the quality of life for asthmatics living on the West Side.

The Western New York Independent Living Center The Independent Living Center offers programs in several areas of concern: individual services, programs for consumers with developmental disabilities, mental health services, support services and community services.
