Assumption of Risk: You understand that there are risks to all physical activities and all precautions are taken to ensure your safety. Fill out this form at the beginning of every semester.

Emergency Card: Provides the school and the instructor with emergency contact and insurance information. Fill out this form at the beginning of every semester.

Driver Agreement: If you would like to volunteer driving other club members to club events, please fill out this form. It provides us with your driver's license number, car license and all those goodies.

Participation Physical Evaluation (New Student Health form): If you are new to the university or have not participated in any university sponsored sports organizations (like clubs), please have this form filled out by a physician. You can have your physician fill this out, or university personnel.

Club Sports History and Consent Form (Continuing Student): If you are continuing with a sports organization, please fill this form out every semester. Unlike the "Participation Physical Evaluation" form, you do not need to have a physician's signature on this form. It asks you a bunch of yes or no questions and you medical history, just to check that you are still physically fit to participate in a sports organization. It also asks for the date of your last physical examination and name of who performed it.

Attendance

  1. Classes will be held three times a week.  We encourage you to attend as many classes as possible, but recommend you be present for a minimum of two classes a week.

  2. Tardiness:  Make all attempts to be to class on time, but unavoidable lateness will occur on occasion.   See the notes on etiquette for coming into class late.  You will have to do a set of push-ups for being late (the number grows with the amount of time you are late!).

  3. If you will be missing more than one week, please speak to (or e-mail) Senpai and let him know of your status.  You are not a number in our classes and you will be missed – the club benefits from each of its members

  4. If you decide to withdraw from the club, please speak to Senpai

Safety

In Seido Karate, safety is #1.  Our Grandmaster, Kaicho Nakamura, stresses safety above all other things.

However, this is Karate.  Injuries are rare, but do occur from time to time.  You participate at your own risk.

Beginning students of Seido Karate are taught the principles of sparring through light-contact drills.  No open kumite is permitted for beginning students.

The University requires students to have personal health insurance.  University students are either covered under a guardian’s policy or it was purchased through your tuition.  Members of the community are recommended to have their own insurance.

As a first semester student, the University requires a pre-participation physical.

It is your responsibility to provide Senpai information regarding injuries, surgeries, medications, or other special considerations which may affect your participation in class.  Immediately tell Senpai of any injuries that occur during the class.

Grading

A physical and written promotional exam will be given the last week of the semester, prior to the University’s final exam week.  You will be evaluated on physical technique and basic knowledge of Seido Karate.  Senpai will provide specific information at that time.  There is a fee required for testing and to continue your training annual dues to the Seido Organization are required.

Attire

Wear clothing that allows freedom of movement.  A gi (karate uniform) is not mandatory for first semester students, but recommended.  A gi with Seido patches is mandatory for continuing students.  A gi and the patches can be purchased in class
