New York State Health Care Proxy Law

In 1991, the second statute recommended by the New York State Task Force on Life became law. The Health Care Proxy law provided a mechanism to make a wide range of decisions for incapacitated patients. By appointing a health care agent, competent adults could empower someone to make decisions for them in the event that they become incapable of making medical decisions. The full text of the Proxy law is available via the NYS Senate gopher, as well as a Proxy Instructions and Form for Patients and Proxy Law Guidebook for Health Care Professionals courtesy of the NYS Department of Health. Health Care Proxy Forms are also available in .pdf format. In order to view or print this version of the document, you will need to have Adobe Acrobat Reader, installed on your computer. You can get a free download of the reader from Adobe Systems Inc or from the local UB software archive.
