REGISTRATION INFORMATION

Please register by completing the attached form ( below) for each registrant. Mail or fax the completed registration form(s) with payment to the address/fax number indicated. Payment may be made by check, purchase order, Visa or MasterCard. Checks and purchase orders should be made payable to UNIVERSITY AT BUFFALO RESEARCH FOUNDATION.  Registration is not considered final until full payment is received. Any questions about courses should be directed to Joseph Syracuse.

Please register early, as class sizes are limited. Confirmations will be sent to those who register at least 10 working days prior to the first day of each course. Confirmations will include course date(s), location, directions, a parking tag, start time and end time. Hotel information will be supplied on request. If you believe you are registered, but have not received a confirmation, please verify your place in class by contacting Joe Syracuse by telephone, fax or email.

Please note: Course fees and dates are subject to change. Registration begins at 8 am, and class begins at 8:30 am.

Course locations and times vary. Please consult your confirmation letter for this information. Students receive a course manual and/or other written materials.

Coffee is also provided in the morning. Certificates of Completion and wallet cards (for most courses) are awarded to students who successfully complete all requirements.

Students are responsible for the purchase of meals and refreshments. Any licensing or credentialing fees are also the responsibility of the student.

CANCELLATION POLICY

The University at Buffalo Toxicology Research Center (UBTRC) reserves the right to cancel programs. In this event, the full registration fee will be refunded. UBTRC, however, is not responsible for travel or other costs incurred by participants.

All cancellations by a registrant must be received in writing and can be faxed, mailed, or e-mailed. Course registration fees will be refunded in full if cancellations are received 11 or more working days before the program start date. Any cancellations or changes, by the registrant, made within 10 working days of the program start date will be charged a $100 administration fee. No-shows and cancellations received on or after the program start date will be invoiced the full amount of the registration fee. The registrant may send a substitute without penalty.
